Quick Guide to Billing and Insurance Claims
- Bring your insurance card to your first appointment.
- At the time of your first or second appointment, you will meet with our Director of Patient Relations who will go over your insurance benefits with you. At this time, she will explain what portion of the bill will be covered by your insurance company and what portion will be your responsibility. She will also review a sample statement with you for clear understanding.
- We will file directly with the insurance company for services rendered.
- Upon receipt of the explanation of benefits form from your insurance company, you will receive notification of patient responsibility.
- For patient convenience, we accept cash, personal checks, money orders, and all major credit cards.
Managed care health plans vary in their restrictions and requirements. To assure highest benefit level of payment, patients need to be aware of guidelines required by their individual plans. Some common requirements your insurance may or may not include:
- Referrals from primary care physicians before patient can be seen by Columbus Physical Therapy, P.C.
- Co-pay and deductible obligations required by patients.
- Prior authorizations before receiving treatment.
- Just tell us what you need and we'll help you get it.
Private insurance coverage may pay for worker’s compensation injuries, on-the-job injuries, motor vehicle accidents, or injuries sustained on personal property where a lawsuit may be filed. In such cases, we will file claims to the insurance companies. If claims are disputed or denied, patients are expected to assume full responsibility for payment.
Our income manager or billing officer will review your case periodically and work closely with your attorneys and case managers.
